When a tyre is punctured is the inner tube always the place with the puncture?

I recently punctured my bike tyre and i have been looking at lots of videos of how tto fix it they all say that the problem is the inner tube but i am sure it is just the outside bit and not the tube that is punctured. Is it completely different to patch the outer bit than the inner tube?
